What is a D-SNP and how does it work?

Dual-Eligible Special Needs Plans (D-SNPs) enroll individuals who are entitled to both Medicare and medical assistance from a state plan under Medicaid. States cover some Medicare costs, depending on the state and the individual’s eligibility. Members receive coverage for their hospital, medical, and prescription drug benefits through one plan. The plan provider helps to coordinate care between doctors, so members get the most comprehensive care available.

How Do I Know If I Qualify for a Dual-Eligible Special Needs Plan (D-SNP)?

To qualify for a D-SNP, an individual must be approved for both Medicare and Medicaid. Person must:

  • be a U.S. citizen or a lawful, permanent resident
  • qualify for Original Medicare (Parts A and B)
  • meet the financial requirements for the person’s state’s Medicaid program
  • live in the D-SNP plan’s service area

What's covered in a D-SNP?

The plan’s Welcome Packet contains important documents like the Over-the-Counter (OTC) catalog and the Summary of Benefits. D-SNPs cover all medically necessary and preventive services offered under Medicare Parts A and B, in addition to prescription drug (Part D) coverage. Many D-SNPs also offer extra benefits that Original Medicare doesn’t include that are listed in the plan's Summary of Benefits.

Members in D-SNPs can join, switch, or drop their plan one (1) time during each 3-month enrollment period. These periods are January to March; April to June; and July to September.
